2002. As far as I know all models except for green Milgauss. Someone else may have more information.

is it hard to notice just looking at it or do you need a magnifying glass?
yea just seem they on my 2 but i still cant see them with the naked eye i had to use my steiner binoculars and look through the worng end of them under a bright light.
My 2004 sub had it, and my 2006 SD has it. You can see it with the naked eye, but, in my experience, I really gotta look at the right angle/lighting to see it.
I don't appear to have it on my 2006 "F" series GMT Master II.
Watching watchers watching watches
Don't judge anything on the Rolex laser etching as the fakes have had that sorted for years now.
Yes, but it's fun to see them.
Take off your watch and hold it close to your face. You will not be able to see it looking at the watch straight on. Instead, tilt it so it's angled slightly away from you with the 12:00 position closest to you (your watch will be nearly horizontal to the floor with the 6:00 tilted down slightly). Stare down across the top of the crystal toward the 6:00. You'll see a slight smudge in the crystal above the 6:00. Watch it long enough and it will resolve into a tiny coronet.
I have heard it's easier to see on dark faced watches, but mine has a light face and I don't have any trouble seeing it. If I can find mine on my 31mm DJ with the naked eye (and at 45 years old with the eyesight issues that advanced age entails) and without a special light, then you guys can find yours on a 40mm watch.
Clarence, my understanding is that the coronet is too noticeable on the green sapphire, so that's why they don't put it on there. Don't know how true that is.

take a small flash light and shine it parallel to the watch crystal. it should make it more visible.
