ID help: Omega w/ Square White Dial - 14k Gold Case & Bracelet

I'm hoping someone can help me identify which model this is and if it's genuine. I've included pics of the dial, case, caseback interior and exterior, and movement. The movement states its 17 jewels. Here are some numbers on the watch:

Caseback interior: There are 2 stamped nos. D6624 & 126802
Movement: There are 2 nos. - One under an Omega symbol - 620 / The second on the outer edge of the movement - 29721617

I'm not sure on the date or if the bracelet is original. My father retrieved it from a pile of salvage gold back in the 70's or 80's and has been in my possession for probably 20 yrs. Any help ID'ing this piece and estimated worth would be greatly appreciated.

Edited: The crown is signed also, however, I forgot to get a photo.
